Case two: Extremely fussy baby and refused to be fed, gastroesophageal reflux
Office visit 05/16/06

1 month old
I have come to my doctor since I was three days old. I’m a little baby girl and I gained almost 2 oz per day for my 1st month of life. I started to have more gas, I started to get fussy, and spit up 1-2 times daily when I was one month old. My parents were told to feed me frequently with small volume, and to position my head and back up after feeding.

6 weeks old
My parents took me back to my doctor when I was 6 weeks old because my parents were concerned that I’m very fussy during daytime, esp. feeding time, and my appetite decreased half from 10 times of breast feeding to 5 times of breasting daily. My parents also pumped breast milk out and fed me with a bottle sometimes, which decreased also from 8-12 oz to 4 oz daily. My mother told my doctor “My baby was very fussy and turned her head to back to avoid to be fed. She spit up rarely, no vomiting, diarrhea, running nose, and no fever. She slept well over night”.

My doctor tried to feed me at her office, I did same things to her by turning my head to side and back, consistently crying, and trying not to get any milk in. Finally, she gave up, and told my parents she consider I probably have the problem of gastroesophageal reflux (The food goes down to the stomach and reflux backup. The acid stomach content can hurt food tract to cause the baby to be fussy and refuse to be fed. Sometimes, the baby doesn’t spit up, because the foods only reflux half way of the food tract.). She prescribed a medication (Zantac) and calculated the doses carefully for me. She told my parents to pay attention to the dosage, which should be 0.6ml (<1.0ml). She also let my parents know the medication usually takes 4-5 days to be effective, and small portion of babies may not respond to this medication well. If that happen to me, she has to change my medication.

2 months old
My doctor found my weight gain is slowing down a little. My parents are concerned that I’m still a fussy, crying girl, but the medication can help my fussiness for about 5-6 hours. My appetite improved from 1.5-2 oz to 3 oz per feeding, but has not reached the volume (4oz) I took when I was one month old. My doctor increased the dosage of medication and asked my parents to take me back for a weight check in one month.

3 months old
I came back for the weight check. I know I gained weight well because I ate and slept well. I’m not a fussy baby any more!

Weight gain April 7 to 17, 2006 gain 5oz for 10 days 0.5oz per day
April 17 to May 16, 2006 gain 24oz for 29 days 0.8oz per day

My doctor told my parents she would like to write a story about me, because I’m the one of the three fussiest babies she saw at this office during last six years.
