Case 3: Failure to thrive

We are the twin sisters. We have come to our doctor since we were 9 days old. Initially we only gained weight half oz daily. After our parents worked with doctor to increase breast feeding and formula feeding, we both gained weight more than 1 oz daily for our 1 st month of life, and then follow normal growing rate.

We missed our 6 month well check-up. We haven’t seen our doctor since 4 month old well check-up.

Weight gain slowed down slightly.

Normal developmental screen.

Weight gain slowed down significantly, which meet the diagnosis of failure to thrive (FTT).

Full developmental delay.

Nasal congestion, cough and wheezing on and off since 4 month old. Has used albuterol 3-4 times daily since two emergency room visits during last 4 weeks.

Doctor's suggestion

Anemias screen (normal) & lead screen (normal) as routine.
Increased solid food
Multivitamin with iron
Enhance learning stimulation

Full developmental delay: Enhance learning stimulation & motor training at home, hearing screen, and referral for developmental screen, physical, occupational, and speech therapy (0-3 program at Illinois).

Persistant reactive airway disease (RAD): Start inhaled steroid and one allergy medication for daily control. Albuterol as needed only.

12 month well check up (11.5 month old)

Our parents didn’t take us back for 2 week follow-up. We both didn’t eat solid food well. My parents told my doctor they fed us 40oz water, 32 oz juice, 40 oz formula, and some table food daily.

Full developmental delay, haven’t got evaluation at 0-3 program. There is progress of speech development.

RAD: Need albuterol once a day for wheezing or congested chest.

Food diary.
Juice 4 oz daily, limits water, instead gives more formula. Switched regular formula (20kcal/oz)to Pediasure (27kcal/oz)
Increased solid food.
Multivitamin with iron

Suggestion and concern:
Come for well check up on time.
Follow-up with primary physician closely after ER visits.
Significant breathing problems can slow down weight gain.
Early learning stimulation at home is good for developmental delay kid and normal kid.
