Today I see three newborn babies who were born in three different teaching hospitals, and they are 4, 5, 7 days old, and all of the three have some feeding difficult and one of them weight loss >10%. I think I should open this page to pass the feeding experience from parents to parents.

Date of birth: weight 7lb2oz (3230gm)
Day 3 of life
: weight 6lb6oz (weight loss >10% of birth weight), discharged from Nursery in the evening.
Day 4 of life (today): weight 6lb3oz (2807gm). I did lose weight 15oz, 13% of my birth weight. In the last 15 hours, I lost 3 oz.
Feeding: breast feeding 12 times a day and adding half oz of formula once a day. I'm a sleepy baby and I can only suck my mother's breast for minutes before falling asleep.
My doctor’s suggestion: Wet warm compress of breasts for several minutes before feeding to dilate the lactate ducts; shorten breast feeding time, 5 minutes for the st one and 5-10minutes for the 2nd breast to keep me sucking the both breasts and save my energy. After breast feeding, add pumped breast milk or formula as much as I need.
Day 6 of life: weight 6lb12oz. I gain 9 oz for last 2 days!!! Breast feeding 8-9 times a day, 5-10minutes each side of breast at the most time. And add formula always after breast feeding, 10 to 50ml each time, 7 times a day (total about 8oz formulas). My mother said that warm compress of breasts and message worked. She only did once and breast milk come. My doctor tells my parents today “Now you are the experts for your baby’s feeding. Keep breast feeding at least 7-8 times a day, and you will see the need for formula adding will gradually decreases.”
My words to the doctor in the Nursery: Please don't discharge other babies if they lost weight >10% on day 3 of their life!!

Date of birth: weight 9lb13oz
Day 5 of life (today): weight 9lb3oz. I did lose weight 10oz, about 6 % of my birth weight.
Feeding: breast feeding 16 times a day, 25 to 30 minutes for each breast. During last 24 hours, I continue crying and sucking my mother's breasts for several hours.
I look a little yellow. My parents didn't get Nursery discharge form and no one instructed them how to follow the yellow skin for me. The only things they know that I was tested for sugar and bilirubin in the hospital, and my mother's blood type is O Rh negative, which means I have opportunity to have blood not matching to my mother's.
My doctor’s suggestion: Shorten breast feeding time, 5-10 minutes for 1st one and 10-15 minutes for the 2nd breast to save my energy. After breast feeding, add pumped breast milk or formula as much as I need. My doctor also tests my blood for bilirubin, which is 15.5mg/dl.
Day 6 of life: My doctor called my mother to follow up my feeding and skin color.
Day 7 of life: weight 9lb6oz. I gain 3 oz for last 2 days. Straight breast feeding 10 times a day, 5-15minutes each side and I can sleep 2-3 hours between feedings. I do have 11 wet diapers which totally weigh 18oz. I have 12.5oz urine for 24 hours (One diaper weigh 0.5oz, 11 diapers weigh 5.50z, 18oz-5.5oz=12.5oz), which means I get 12.5 oz milk for last 24 hours. My doctor concern I may need a little more milk. I look more yellow, and even my eye turns yellow.
My doctor’s suggestion: "It is so good gaining 3oz in 2 days. Since the baby has significant jaundice, I will send baby for checking stat bilirubin and other tests, and will closely follow up tomorrow for the jaundice and baby's weight." My bilirubin is 17.2.
Day 8 of Life: weight 9lb9oz. I gain 3 oz for last 24 hours. Breast feeding 10 times a day. I sleep well. Since I'm strong baby, my parents decide to let me suck breasts a little long time as 15 to 20 minutes. Yes, I do well. My yellow skin is getting better, and bilirubin is 16.0.
My words to the doctor in the Nursery: Please give all babies' parents the Nursery Discharge forms which include mother's perinatal lab results, labor and delivery status, babies' birth weight, length, head circumference, medical conditions, and how to follow-up. And please instruct parents well for following up the medical conditions.
My words to the all parents: Now you know you need to ask and get the Nursery Discharge forms and remember to take them to our doctors. And always take your babies to their doctors in 2-3 days after discharging from Nursery to follow up feeding, weight gain, and yellow skin. The text book said "The newborn's weight may drop 10% below birthweight in the 1st wk..." But my doctor told my parents "The majority of the babies came to her office lost weight for the 1st 3-4 days of life, and weight loss usually < 7% of their birth weight. And they should gain weight 1-2oz per day after day 3-4 of their life. " I was so hungery and fussy when I lost 6% of my birth weight on the day 5 of my life.

Day 2 of life
: weight 5lb2oz (2325gm), discharged from Nursery
Day 3 of life: The baby was sent to a clinic instead of the baby's primary physician's office. Weight?
Day 7 of life (today): weight 5lb4oz (2381gm). Gain about 2oz in last 5 days.
Feeding: I have breast feeding 8-10 times a day, 15 minutes each side. But I only had 1-2 wet diapers and cried a lot on day 5 of my life. My mother carried me with my Dad to the infant care class before I born. We all know the baby should have 6-8 wet diapers a day after day 3 of life. A strong sucking and crying baby, with only 1-2 wet diapers a day, is a hungry baby. On the day 6of my life, my parents added 0.5 to 1 oz formula after each breast feedings and my wet diapers increased to 8 times a day in the next 24 hours.
My doctor’s suggestion: Wet warm compress of breasts for several minutes before feeding to dilate the lactate ducts; shorten breast feeding time if no swallowing sounds for last 5 minutes of feeding to save my energy; and continue to add formula as much as I want after breast feedings.
Day 10 of life: weight 5lb10oz, gain weight 6oz for last 3 days.
Feeding: Breast feeding 10minutes each side, 8-10 times a day, with 1-1.2oz formula adding after each breast feedings. I slept well for 2-3 hours between feedings. I have 9 heavy wet diapers for last 24 hours, which weigh 1lb 2.9oz (18.9oz). So I got 14.4 oz milk for last 24 hours which just fit what I need about 2.5oz per pound of weight. (Each dry diaper weigh 0.5oz. 18.9oz - (0.5 x 9diapers) =14.4oz)
My words to the doctor in the Nursery: Please send me to my doctor directly, so my doctor can provide continuity care for me.
My words to the all parents: Please learn some feeding concept and skills as my parents before we born, so you can catch the signs of hungry babies and make right decision to feed us. Thanks my mom and dad to feed me as I need.

Birth weight: 3800 gm. Went home on day 2 of life, weight 3810 gm.
The first doctor’s visit on day 5 of life, weight 3629gm, lost 6 oz since discharged from hospital. Mother’s breasts are engorged. But there are only several drops of milk hanging over her nipples when squeeze the breasts.

Mother: My baby cried a lot. I worry about my baby is hungry.

Dr Shi:
The full-term newborn baby usually has weight loss for initial three days of life, then gains weight 1-2 oz per day. The lost weight usually recovers around day 7 of life.
Your baby is pink, active, suck strongly, breath comfortably, and no fever. He doesn’t have signs of infection. He is most likely crying for hungry. Engorged breast tell me you have enough milk. The drops of breast milk hanging on the nipple when squeezed tell me the lactating ducts (tubes connect breast lobes to nipple) may not open well.
I suggest applying wet warm towel to breast and message the firm area of the breast several minutes before breast-feeding. This will help to open up the lactating ducts. When your baby sucks nipples, you or your husbanded can squeeze the base of breasts to help to open the lactating ducts and push milk out. I would like to get weight check in 2-3 days.

"Our baby has had too much gas, fussy, and too little sleep during the day time since 2 weeks old which have been getting worse for last two days. And she starts spitting up 1-3 tsps of milk after each feeding. She eagers to eat, but holds head back, pulls her legs up, and cries a lot when she is fed. Her bowel movements are normal."

Total sleep time is about 8-11 hours per day.

Daytime: Only sleeps one hour each time with holding on mother's arms, then has fussy for 1-1.5 hour.
Evening and night time: She is exhausted and sleeps 2-3 hours between the breast feedings from 10pm to 5am.
Dr: " Most likely Gastroesophageal Reflux ".
Plan: Hold her upright for 30-40 minutes after feedings. Gives her frequent small feedings. Consider to give her medication if the symptoms continue.

I still spit-up 1 tea spoon or 1table spoon milk right after each feeding or 2 hours after feeding. I'm fussy and have some congested sound. My doctor starts Anti-reflux medication for me.

On the medication, my spit-up is getting better with one spit-up per day. I can sleep 2-3 hours after each feeding over the night. But I'm fussy all day along and only can sleep half hour here or there. I only gain 3 oz during last 10 days. I should gain almost 1oz per day. Oh, please help me. My parents and doctor talk for long time, and finally they decide to discontinue the anti-reflux medication, give me breast feeding at least 8 times. After each breast feeding, pump each breast and feed me the pumped breast milk as I want. Then I will be followed with my doctor in three days. My parents will bring the feeding log and my 24 hours diapers to the clinic.

Wow, I gain 5 oz for last three days. I sleep 3 hours after each feeding over night, and sleep half to one and half hour after playing 2-3 hours during day time. My total sleeping time is about 12-13 hours a day. My mother breast feeds me 8-9 times a day, and adds about 20-30ml pumped breast milk on the 1st 2 days. Then I refused the pumped breast milk after breast feeding. There are 11wet diapers my parents changed for me for last 24 hours, and they weigh 1lb12oz. My doctor told my parents I sucked in 22.5oz from the breasts for last 24hours. (1lb12oz - 5.5oz=22.5oz, each dry diaper weigh half oz). Now I'm 9lb.
Tip: The most of the babies during their 1st year of life need about 50 calories per one pound weight daily to gain weight. One ounce breast milk or formula =20 calories. So babies need about 2.5oz (50 calories) breast milk or formula per pound daily. 6 lb baby needs to be fed 15oz milk, and 9lb baby needs 22.5oz milk to gain weight.
